Classifying disease: infectious, non-communicable and deficiency
Disease divides into infectious (communicable) disease caused by pathogens and spread by contagion or a vector, non-communicable (degenerative) disease that is not passed on, and deficiency disease caused by lack of a nutrient — a classification you must be able to apply to named diseases.
14.1.1 starts by CLASSIFYING disease. Examiners want you to place named diseases into the correct category and to know HOW each spreads — this underpins every later answer in Topic 14.
Infectious (communicable) disease. An infectious disease is caused by a pathogen — a disease-causing organism, namely a bacterium (e.g. tuberculosis, cholera), a virus (e.g. measles, COVID-19, HIV), a protozoan (e.g. malaria) or a fungus. Infectious diseases are passed on, and this happens in one of two main ways:
- Contagious / directly transmitted — passed from person to person through the air (coughs, sneezes), touch, contaminated water or food, or bodily fluids. Examples: tuberculosis, measles, cholera, COVID-19.
- Vector-borne — carried from host to host by another organism (a vector), most often an insect. Examples: malaria and dengue (mosquitoes), sleeping sickness (tsetse fly), schistosomiasis (water snails). Vector-borne diseases are tied to the vector's habitat, which is why they are so concentrated in warm, wet regions.
Non-communicable (degenerative) disease. A non-communicable disease (NCD) is NOT passed from person to person. It develops gradually, often over years, through ageing, lifestyle and genetics. The main NCDs are cardiovascular disease (CVD) (heart disease and stroke), cancers, chronic respiratory disease and diabetes. They are also called degenerative diseases because body systems gradually break down. NCDs are strongly linked to ageing populations and to lifestyle risk factors (poor diet, smoking, alcohol, inactivity), so they dominate in HICs.
Deficiency disease. A deficiency disease is caused by a LACK of an essential nutrient in the diet — not by a pathogen and not passed on. Examples: kwashiorkor and marasmus (protein–energy deficiency), rickets (vitamin D), anaemia (iron) and scurvy (vitamin C). These are most common where poverty and poor nutrition prevent a balanced diet.

The take-home for 14.1.1: classify by both the CAUSE (pathogen vs degeneration vs nutrient lack) and, for infectious disease, the MODE of spread (contagious vs vector-borne) — and always anchor it to NAMED diseases such as malaria, TB, cardiovascular disease and kwashiorkor.
